Periodic Credit Report Authorization

In connection with your Tally Premium membership and in order to prequalify and/or evaluate your eligibility for certain credit programs with Tally Technologies, Inc. (“Tally”) or its financing partners, Tally requests authorization to periodically request your consumer credit reports from a consumer reporting agency. This does not impact your credit score.

You understand that by enrolling in Tally Premium, you are asserting that you:

• Hereby provide “written instructions” under the Fair Credit Reporting Act authorizing Tally to periodically obtain information about your personal credit report from a consumer reporting agency. 

• Authorize Tally to obtain such information solely to conduct an evaluation and/or prequalification of your eligibility for certain credit products with Tally.

• Acknowledge that you understand that Tally’s request for your consumer report may appear on the inquiries section of your consumer report.  

• Acknowledge that upon your request, submitted to help@meettally.com or P.O. Box 411165, San Francisco, CA 94141, Tally will inform you whether or not a consumer report was requested, and if such report was requested, Tally will also inform you of the name and address of the consumer reporting agency that furnished the report.

• Understand, acknowledge, and agree that your consent for authorization for Tally to periodically request consumer credit reports will remain in effect until the earlier of when (1) you are no longer a Tally Premium subscription holder, or (2) you notify Tally at help@meettally.com or P.O. Box 411165, San Francisco, CA 94141 that you are revoking your consent.

AUTHORIZATION PURSUANT TO THE

CALIFORNIA CONSUMER CREDIT REPORTING AGENCIES ACT 

(CA CIVIL CODE SECTIONS 1785.1, ET. SEQ.)

By enrolling in Tally Premium , you assert that:

You have carefully read and understand the California Consumer Credit Reporting Agencies Act Disclosure and Authorization forms. You consent to the periodic preparation of consumer credit reports by a consumer reporting agency, and to the release of such reports to TALLY TECHNOLOGIES, INC. (“Tally”) and its designated representatives and agents, for the purpose of assisting Tally or its financing partner(s) in prequalifying and/or evaluating your eligibility for certain credit products. 

You authorize consumer reporting agencies to disclose any information about or concerning me to Tally and its agents. The information that can be disclosed to Tally and its agents includes your credit history, and by accepting the terms and conditions, You agree this consent will be valid for any consumer credit reports that may be requested by or on behalf of Tally.

You further understand, acknowledge, and agree that this consent for authorization for Tally to periodically request your consumer credit reports will remain in effect until the earlier of when (1) You are no longer a Tally Premium subscription holder, or (2) You notify Tally at help@meettally.com or P.O. Box 411165, San Francisco, CA 94141 that you are revoking this consent.
